ABSTRACT :

  

Significant piping failures caused by flow-accelerated corrosion (FAC) continue to occur, even after the Mihama Power Station unit 3 secondary pipe rupture in 2004, in which five contract workers died and six were seriously injured. All reactor types have experienced FAC-related events in various piping systems. Events include a feedwater recirculation line failure, a steam return line that ruptured downstream of the first stage of the moisture separator reheater, and failure of an extraction steam line bellows inside the condenser. In addition, FAC has affected some pressure vessels, such as the rupture of a low pressure feedwater heater shell at one station. These FAC-related events have caused sudden ruptures of high- and moderate-energy systems, resulting in plant transients. Additionally, the steam and water leaks affect safety and non-safety related equipment and also pose personnel safety concerns.

An analysis of about 40 FAC events between 1999 and 2007 indicates five main categories of problems:

  1. Inadequate FAC monitoring programme
  2. Adverse effects of operating conditions or plant configuration change
  3. Secondary pressure vessel degradation
  4. Software knowledge deficiencies and inadequate modelling

After the accident at the Chernobyl nuclear power plant in 1986, nuclear operators world-wide realised that the consequences had an effect on every nuclear power plant and international cooperation was needed to ensure that such an accident can never happen again.
WANO was formed in May 1989 by nuclear operators world-wide uniting to exchange operating experience in a culture of openness, so members can work together to achieve the highest possible standards of nuclear safety.

The culture of openness allows each operator to benefit and learn from others' experiences, challenges and best practice, with the ultimate goal of improving nuclear plant safety, reliability and performance levels for the benefit of their customers throughout the world.
